If the effective depth is greater than 36 inches then skin reinforcement is
required per 10.6.7 of ACI.
You're probably already aware of that provision if you're used to working
with fairly deep beams. But, it's always seemed like an obscure, easy to
overlook provision to me.

I don't have my ACI in front of me, and I don't have real world design
experience with this type of a concrete beam. So, I may be completely wrong.
But, I recall there being a "skin reinforcement" requirement in ACI that
would get triggered by a beam of this depth? I don't recall all the
details, but when I last studied the concept (and concluded that I didn't
need skin reinforcement) I made a mental note that it would apply to beams
like this.
