I had looked at that section in AISC Design Guide 1 (2nd Edition), but the 1/2" they refer to is "from the edge of the hole to the edge of the plate" when not subjected to lateral load. For the clearance between the washer and the column/ weld, it just says to provide "ample clearance"?
